KGI Campus Visitor Form

In order to mitigate the possible transmission of COVID-19, Keck Graduate Institute (KGI) is taking extra precautions with safety protocols and policies for KGI students, faculty, staff, and campus visitors.

CONDITIONS FOR CAMPUS ENTRY

By entering the KGI campus, you agree to the following terms:

You must be free of COVID-19 symptoms and may not have had contact with a positively diagnosed case of COVID-19 in the past 14 days
Per our COVID-19 Vaccination Policy, students, staff, and faculty are required to be vaccinated against COVID-19. Guests visiting campus for more than 8 hours in one day must submit proof of COVID-19 vaccination as directed below. If you are not vaccinated and visiting campus for more than 8 hours in one day, we will require proof of a negative COVID test within 48 hours of arrival on campus. Please submit your proof of negative test below. All exemptions should go through the Human Resources Office at hr@kgi.edu. If you are visiting campus for less than 8 hours in one day, you do not need to upload proof of vaccination.
Upon arrival, please get in touch with your KGI contact so they can provide access to the campus buildings.

Based on your answer, we are prohibiting you from visiting campus. We encourage you to follow up for a visit when more than 14 days have passed since your positive test and you are completely symptom free for that same length of time.

Have you had contact with a person known to be infected COVID-19 in the last 14 days?*

Based on your answer, we are prohibiting you from visiting campus. We encourage you to follow up for a visit when more than 14 days have passed since your last contact with a COVID-19 and you are completely symptom free for that same length of time.
